Periodontal disease is the progression of gingivitis (inflammation of the gums) to the point of infection of the supporting bone.  This results in loss of bone support.  At this point it is considered periodontitis, the most common cause of tooth loss.  The treatment involves deep cleaning of the teeth as well as laser treatment and possibly antibiotics.
